Rental agreement

Standard landlord / tenant agreement. Stays while service needs can be met within assisted living scope.

Service plan

The tenant is assessed to identify need and the supports available within the service. Once identified, a service plan is developed and an annual assisted agreement is signed. The agreement outlines the scope of assisted living services, and responsibilities of the provider and the tenant.

The service delivery model requires a level of self-sufficiency where only minor help is needed to be successful when assisted with:

  • Medication prompts
  • Bathing
  • Dressing
  • Laundry
  • Light housekeeping
  • Attendant support

Eligibility

Eligibility depends on the specific needs of the client as assessed by Ontario Health atHome.
