Request EMS records

Niagara Emergency Medical Services can provide ambulance call reports, dispatch summaries and 911 call recordings. Request forms and requirements vary depending on who's making the request.

Types of records

Emergency Medical Services can provide:

  • Ambulance call reports
  • Dispatch summary reports
  • Audio recordings of incoming 911 calls to Niagara EMS

How to request records

An application fee may apply when you submit a request. The fee cannot be waived. Allow up to 30 days for processing from the time the form and payment are received. You will be notified if more time is needed.

Before starting your records request, make sure you have:

  • Government photo identification
  • If you're not the patient, proof of legal authority for substitute decision making, such as power of attorney or will

Once we find the records, we'll send you an invoice for the processing fee. Contact us for a paper application.

Request release of patient information.

To submit a request, mail or email nemsrecords@niagararegion.ca the request on company letterhead. You must include:

  • Patient's name
  • Patient's date of birth
  • Date, time, and location of the incident
  • The hospital the patient was transported to, if applicable
  • Signed patient authorization form

This form is to be completed for all personal information requests received by Niagara Region from the Niagara Regional Police Service or any group with the legal authority to conduct a law enforcement investigation. A formal request under the Municipal Freedom of Information and Protection of Privacy Act (MFIPPA) is not needed.

If a formal Freedom of Information request under MFIPPA has already been submitted through the Clerk's Office, this form does not need to be completed.

MFIPPA allows the sharing of personal or confidential information with agencies that have legal powers for law enforcement. This includes:

  • Policing
  • Investigations or inspections that could lead to court or tribunal proceedings where penalties or sanctions may apply
  • The conduct of proceedings related to investigations or inspections

If you're requesting records on behalf of an organization with special legislative authority, we may ask your organization to provide a letter citing the relevant authority to support your disclosure request.

Request law enforcement investigation information.

Personal health information of deceased patients

For disclosure requests related to the personal health information of deceased patients, consent must be received from the estate trustee or the individual who has assumed responsibility for administering the deceased's estate, if there's no trustee.

Proof of the requestor's authority to consent to the disclosure of the records must be submitted — for example, a copy of the last will and testament or a Certificate of Appointment of Estate Trustee without a will.
