    • Two pieces of government issued identification. One piece of ID must be photo identification, such as an Ontario driver's licence, Canadian passport or Ontario photo card.
    • Must be over 18
    • If born outside of Canada, confirmation that applicant is legally entitled to work in Canada, such as a Canadian passport, permanent resident card, work permit (no social insurance cards)
    • Letter from employer confirming employment
    • Criminal record and judicial matters check obtained within 60 days
    • Colour photograph (headshot, blank background, no hats or glasses) taken within six months

    • Renewal - $50

    • Verification of business model (corporation, partnership, registered business)
    • Identification of all who have signing authority for the business
    • Criminal records and judicial matters check obtained within 60 days
    • Colour photograph (headshot, blank background, no hats or glasses) taken within six months
