Applications for Vendors at Special Events and Markets

Vendors at public events and markets are required to follow health guidelines for:

  • Preparing food
  • Serving food (pre-packaged or prepared on-site)
  • Tattooing
  • Piercing
  • Micropigmentation

These rules decrease the risk of illness and disease that can occur at these events.

Vendors must complete the proper form(s) and submit to the event organizations, who will mail or fax all the forms to us.

Food and Farmers Market Vendors

  • Food Vendor Package
    Guidelines and application form for vendors selling food in booths, trailers, carts at festivals/events. Form must be given to the event organizer at least two weeks before the event.
  • Farmer's Market Vendor Package
    Guidelines and application form for vendors selling food at markets, either pre-packaged or prepared on-site. Form must be given to the event organizer at least two weeks before the event.

Tattoo and Piercing Vendors (Personal Services)

  • Personal Services Vendor Package
    Guidelines and application form for vendors offering tattooing, piercings or micropigmentation. Form must be given to the event organizer at least two weeks before the event.

Event Coordinators

  • Event Coordinator Package
    Guidelines and application form for coordinators of special events. Form must be mailed or faxed to us at least one month before the event.
