Niagara Ontario Health Team - Équipe Santé Ontario Niagara

Background

The Ontario Ministry of Health announced a new vision for health care. A key element of their strategy is setting up Ontario Health Teams, or OHTs. The teams are groups of care providers and organizations that are clinically and financially accountable for delivering a full and coordinated continuum of care to a defined population.

The goal is a health care system where patients, clients, families and caregivers can seamlessly transition between services, such as:

  • Moving in or out of hospital
  • Coordinating home and community care with their family doctor
  • Transitioning to long-term care

Benefits would include having better access to health care records and being active partners in care decisions.

The Process

The evolution to Ontario Health Teams is happening in stages.

The ministry asked health care providers and agencies across the province to complete a self-assessment. It outlined how they can work together to form an Ontario Health Team in their geographic area.

  • 157 self-assessments were submitted from across the province
  • 31 teams were invited to submit a full application
  • 43 teams were invited to continue developing their self-assessments to move to a full application in the future

The Niagara Ontario Health Team - Équipe Santé Ontario Niagara was one of the 31 teams invited to submit a full application.

On Oct. 8, 2019, the team submitted the full application to the Ministry of Health. Within the team, there are over 44 organizational partners, 60 primary care physicians and 6 patient / client, family and caregiver advisors.

Next Steps

Full application submissions will be evaluated by third-party reviewers and the Ministry of Health. It will be evaluated against standard criteria that reflect the readiness and ability of teams to successfully implement the model and year 1 expectations. After the evaluation, teams will either be:

  • Invited to move to the final stage of evaluation, or
  • Continue to work towards readiness as a team in development

Those teams most ready to move to the final stage of evaluation may be invited to participate in community visits. This will further inform the final selection of the first cohort of Ontario Health Team candidates.
