Admission to Long-term Care Homes

Hamilton Niagara Haldimand Brant - Community Care Access Centre is responsible for applications to long-term care homes in Niagara.

They determine your eligibility and priority for admission, and manage the waiting lists for homes. To be eligible, applicants must:

  • Need on-site 24-hour nursing care
  • Need assistance with daily living activities
  • Be 18 years of age or older
  • Have a valid Ontario Health Card

The Access Centre can provide you with information about local homes and their services as well as things to consider when you are choosing a home.

Cost

The homes offer three different accommodations: basic, semi-private and private. Accommodation rates are set by the Ministry of Health and Long-term Care.

You may qualify for a rate reduction if the cost of basic accommodation is beyond your means.

If you are a veteran, you may qualify for extra financial help.
