Convalescent Care

Services are temporarily unavailable due to COVID-19.

The convalescent care program provides quality care to individuals who need time and support to recover before returning home from a hospital visit. Eligibility is determined by the hospital discharge planner.

The program meets the health care needs of clients and reduces pressure on crowded hospitals.

Niagara's convalescent care program can accommodate up to 20 clients at one time. Individuals in the program have access to a range of services provided by on-site professionals, including:

  • Family physicians
  • Nurse practitioners
  • Registered nurses and registered practical nurses
  • Personal support workers
  • Physical and occupational therapists
  • Social workers

Length of stay

Clients may stay up to a maximum of 90 days per year. The expected length of stay in the program is between 30 and 45 days.
