Case Management Services
We offer case management services for residents. These services focus on providing support to individuals with serious, ongoing mental health needs.
Clinical Case Management
To be eligible for the Region's clinical case management services, individuals must meet the following criteria:
- Suffer from ongoing and serious mental illness
- Have difficulty managing medication and their side effects
- Need help coordinating multiple services
- Need help with life skills and activities of daily living
- Require education and counselling for themselves and their family

Referral Process - What to Expect

Step 1: The initial telephone call
An intake coordinator will answer calls Monday to Friday, from 8:30 a.m. to 4:30 p.m. If you leave a message for a call back, all information is kept confidential.
The purpose of this call is to ensure that the person being referred meets the criteria for the Case Management Mental Health program.
If making a referral for someone else, when possible it is a good idea for the person being referred to be aware of and/or involved in this initial conversation.

Step 2: Evaluating the Referral
The initial information about the referral is evaluated by Intake Worker and Clinical Manager.

Step 3: Contacting the Referred Person
If the referral criteria is met, we will contact the individual to gain a better understanding of the person's situation and ensure they consent to the referral and to discuss options available through the program.

Step 4: Initial Meeting
The initial meeting may take place in a variety of locations such as our office, a coffee shop, or an individual's home.

Step 5: Counselling/Treatment
The individual will be partnered with a staff person for ongoing treatment, education, and support.
