Gonorrhea Treatment Guide for Doctors

Recommended Regimens

  • Ceftriaxone 250 mg IM in a single dose and Azithromycin 1 g PO

Alternative

  • Azithromycin 2 g PO in a single dose

During Pregnancy

Same as recommended treatment.

Penicillin Allergy

Same as recommended treatment regimen, except the use of cephalosporins is contraindicated in persons with an IgE mediated penicillin allergy (e.g. anaphylaxis, Steven Johnson’s syndrome).

  • 2 g Azithromycin PO in a single dose

Follow-up

Rescreen six months post treatment for potential reinfection.

Culture four days or more post treatment if:

  • Alternative treatment used
  • Suspected or confirmed treatment failure
  • Pregnancy
  • Pharyngeal or rectal infection

NAAT two weeks or more post treatment if this is the only option.
