Reporting COVID-19 Cases for Health Care Settings and Congregate Living Sites

This reporting form must be completed when there is a confirmed case of COVID-19 connected to a health care setting or congregate living site. This includes:

  • Health care providers
  • EMS
  • Long-term care homes
  • Retirement homes
  • Home health care
  • Chiropractor
  • Registered massage therapist
  • Dentist
  • Shelters
  • Group homes
  • Correctional facilities
  • Children or youth residential settings

Information collected by this form will be used to support case management and contact tracing by Public Health.
